The purpose of this project is to improve collaboration and share best practice to improve the quality of life of people living with dementia and their carers. It builds on some of the work of the Dementia Services Development Centre — projects such as Dementia and Imagination, Individual Cognitive Stimulation Therapy (iCST), and Cognitive Function and Ageing Study Wales (CFAS).

This project aims to create a dementia supportive practice community (network) in North Wales by sharing best practice, improving collaboration between research, services and people affected by dementia, and increasing the impact of dementia research into practice via a series of meetings and workshops. Practice is developed on co-production with a range of people who are ‘experts by experience’. This includes people living with dementia; carers; care and support staff; researchers; and healthcare professionals.

The project provides an opportunity to share information and ideas, voice concerns, form new partnerships and to ensure that the voices of people living with dementia are heard to ultimately establish a dementia supportive network.

Diben y prosiect hwn yw gwella cydweithio a rhannu arferion gorau er mwyn gwella ansawdd bywyd pobl sy’n byw gyda dementia a’u gofalwyr. Mae’n adeiladu ar beth o waith y Ganolfan Datblygu Gwasanaethau Dementia – prosiectau megis Dementia a’r Dychymyg, Therapi Ysgogi Gwybyddol Unigol (iCST), ac Astudiaeth Gweithrediad Gwybyddol a Heneiddio Cymru (CFAS).

Bwriad y prosiect hwn yw creu cymuned ymarfer (rhwydwaith) sy’n gefnogol o ddementia yng Ngogledd Cymru trwy rannu’r arferion gorau, gwella’r cydweithio rhwng ymchwil, gwasanaethau a phobl y mae dementia yn effeithio arnynt, a gwella effaith ymchwil dementia ar ymarfer trwy gyfrwng cyfres o gyfarfodydd a gweithdai. Caiff yr ymarfer ei ddatblygu trwy gyd-gynhyrchu gyda nifer o bobl sydd yn ‘arbenigwyr trwy brofiad’. Mae hyn yn cynnwys pobl sy’n byw â dementia; gofalwyr; staff gofalu a chefnogi; ymchwilwyr; a gweithwyr iechyd proffesiynol.

Mae’r prosiect yn darparu cyfle i rannu gwybodaeth a syniadau, lleisio pryderon, ffurfio partneriaethau newydd a sicrhau fod lleisiau pobl sy’n byw â dementia yn cael eu clywed er mwyn sefydlu rhwydwaith sy’n gefnogol o ddementia maes o law.
